What is Form SSA-1696?

Filling out Form SSA-1696 is a very important step in the application process, if you are getting help with your claim. The SSA-1696 is a document that allows a claimant (someone who wants to apply for benefits) to appoint a representative to assist them with their application process. To have someone represent you the way Citizens Disability does for our clients (handling your application materials, dealing with all the paperwork, staying on top of deadlines, and appearing with you at any hearings to decide your case) you’ll need to fill out a form SSA-1696 and submit it to the Social Security Administration.
